In order to begin the process for enrollment consideration into one of PrepMD’s training sessions, download and completely fill out the (pdf) Enrollment Application requested above.  In addition you must…

  1. Provide all background information requested including detail on US citizenship.
  2. Submit a copy of your transcript from an accredited 4 year Baccalaureate Degree school.
  3. Ensure that you have provided (on separate paper) the short and long essays regarding your interest in engaging this unique educational opportunity.
  4. Include a copy of your CV/resume along with the Enrollment Application.
  5. Provide at least 2 letters of reference from former employers, professors, or colleagues.
  6. Submit proof of your personal healthcare insurance that will coincide with your attendance at PrepMD.
  7. If you will have a car while attending PrepMD training, submit a copy of your driver’s license and your automobile insurance that will coincide with your attendance.
  8. Include a check as remittance for the Enrollment Application Fee in the amount of $100.00.*

* This fee is non-refundable and will be applied to cover the expense of background checking services on all student candidates.
